PREPARING FOR  YOUR EVALUATION

Call 321-972-3246 or email christine@psycheec.com to schedule your appointment. Once your appointment is scheduled, please review the following steps carefully to ensure that you are prepared for your appointment:

 

  1. You will be emailed a link to a portal that contains important documents to sign. Please ensure that you complete all of the online paperwork prior to your appointment.

  2. Medical and psychological records are very important in the evaluation process and can greatly assist in giving a more accurate and specific diagnosis. If  the person being evaluated has an IEP (Individualized Education Plan) at school, this is also very helpful information to provide. Getting those records to PscyhEEC prior to the evaluation is best so that they can be reviewed ahead of time.

  3. Please arrive (or be in the online waiting room, if you are being seen through the video conferencing system) 10-15 minutes prior to your scheduled appointment. If you are more than 10 minutes late, please understand that we may need to reschedule. If you need to reschedule for any reason, please let us know at least 24 hours in advance. If you cancel or change your appointment within one to six hours of the appointment time, you will be charged a $75 nonrefundable fee. 

  4. Bring identification with you to your appointment.

  5. If the individual being tested takes medication, ensure that they take the prescribed medication on the days leading up to the appointment and on the day of the appointment. However, if you are prescribed a PRN (i.e. “as needed”) medication, such as a benzodiazepine or pain medication that is a narcotic, it is recommended that you do not take that medication on the day of testing unless absolutely necessary. If you do have to take it, make sure to let the psychologist know. BRING ALL YOUR MEDICATIONS WITH YOU TO YOUR APPOINTMENT IN THEIR ORIGINAL BOTTLES. If you are being seen online, have the bottles easily accessible. 

  6. Make sure to eat a full, healthy meal prior to the appointment. You're welcome to bring something to drink and snacks with for your appointment. Particularly for children being tested, avoid food and drinks with added sugar and caffeine.

  7. Payment for services are required up front through the Theraplatform system through a debit, credit, or HSA card. If you arrive at the appointment without sufficient funds to cover the full cost of the appointment, the associated late cancellation/change fee applies. If you need to pay in cash, let us know so that arrangements can be made. For self-pay, half of the cost is due at the initial online appointment, and the other half of the cost is due at the time of testing.

  8. If the person being tested wears glasses, a hearing aid, or any other assistive devices, ensure that those items are also brought to the appointment.

  9. Testing is done with only the person being tested in the office, regardless of whether an adult or child. So if your child is being tested, you’ll need to remain in the waiting room for the duration of the testing. If you also need to bring other children with you who are not participating in the testing, it is recommended that you have plenty of things to keep them occupied for an extended period of time.
